Hidden IC Food Triggers: Citric Acid, Sweeteners, Preservatives and Seasonings

Quick answer: which hidden ingredients may trigger IC symptoms?
Packaged foods can contain acids, sweeteners and seasonings that are easy to miss when you only look at the product name. For some people with interstitial cystitis/bladder pain syndrome (IC/BPS), useful ingredients to investigate include citric acid, added vitamin C, artificial sweeteners, MSG, chilli, tomato powder, citrus, vinegar and strongly flavoured seasoning blends.
Preservatives and E-numbers as a whole are not proven universal IC triggers. An E-number identifies an authorised food additive; it does not tell you whether it will affect your bladder. The aim is to find a repeatable personal reaction, not to fear every unfamiliar ingredient.

Why a “safe” food can still cause a flare

You may tolerate a plain ingredient but react to a prepared version. Plain chicken may be comfortable while seasoned chicken contains chilli, lemon, vinegar and flavour enhancer. Plain yoghurt may be fine while a flavoured yoghurt contains citric acid, fruit concentrate and sweetener.
This does not prove that one additive caused the reaction. Packaged products often contain a trigger stack—several possible irritants in the same serving.
What does the evidence support?
NIDDK lists artificial sweeteners and MSG alongside citrus, coffee, tea, fizzy drinks, alcohol, tomatoes, spicy foods and chocolate as commonly reported IC triggers. Food-sensitivity questionnaires also identify artificial sweeteners, vitamin C and acidic or spicy products as common concerns.
Most of this evidence is based on self-reported symptoms, not blinded food challenges. A 2026 systematic review found individualised elimination promising, but only four studies with 147 participants met its criteria. The evidence is too limited for a universal additive-free IC diet.
How to read a UK ingredients list
Ingredients on prepacked foods are generally listed in descending order by weight. Additives may appear by name or E-number, often after their function—for example, “antioxidant: ascorbic acid” or “antioxidant: E300”.
Read the ingredients list rather than relying on claims such as “natural”, “no added sugar”, “low calorie” or “vitamin enriched”. A short list is not automatically healthier, but it is usually easier to test.
Citric acid, vitamin C and other acids
Citric acid: E330
Citric acid is used in drinks, sweets, sauces, yoghurt, canned foods, supplements and fruit-flavoured products. It is chemically different from citrus fruit, although both can add acidity.
Look for citric acid or E330. Sodium citrate (E331), potassium citrate (E332) and calcium citrate (E333) are related salts but are not identical to citric acid. Do not assume they will affect you in the same way.
Ascorbic acid or vitamin C: E300
Ascorbic acid is vitamin C. It is not the same as citric acid. Vitamin C was associated with symptom worsening in a large IC dietary questionnaire, but dose and formulation may matter.
It may appear as ascorbic acid, vitamin C or E300. Sodium ascorbate (E301) and calcium ascorbate (E302) are buffered forms, but they are not guaranteed to be comfortable.
Other acids
You may also see malic acid (E296), acetic acid (E260), phosphoric acid (E338), tartaric acid (E334) or lactic acid (E270). These are chemically different. Instead of avoiding every acid, compare an acidic packaged product with a simpler version of the same food.
Artificial and intense sweeteners
Common label terms include aspartame (E951), acesulfame K (E950), saccharin (E954), sucralose (E955), cyclamates (E952) and steviol glycosides or stevia (E960).
Products may combine several sweeteners, especially diet drinks, flavoured water, chewing gum, sweets, yoghurt, protein products, medicines and supplements. Research does not prove that every sweetener has the same effect, so test products individually.
Sugar alcohols such as sorbitol, xylitol, maltitol and erythritol are not established IC triggers, but they can cause bloating or diarrhoea in some people. Gut symptoms can then complicate pelvic and bladder symptoms.
MSG, yeast extract and flavour enhancers
MSG may appear as monosodium glutamate, MSG, E621 or “flavour enhancer: E621”. Other glutamate salts include E620 and E622–E625. E627, E631 and E635 are additional flavour enhancers often used in crisps, instant noodles, stocks and seasoning powders.
Yeast extract is not the same ingredient as added MSG, although it naturally contains glutamate and gives a savoury taste. There is not good IC-specific evidence proving that yeast extract or hydrolysed protein causes the same reaction as MSG. Test them separately rather than automatically treating them as hidden MSG.
Seasonings, marinades and stock products
Check seasoning blends for chilli, cayenne, tomato powder, citrus peel, lemon or lime juice, vinegar powder, citric acid, malic acid, MSG, sweeteners and concentrated fruit powders.
Paprika, black pepper, garlic, onion and curry spices are not universal triggers. Test individual seasonings before avoiding every spice.
Stock cubes, gravy granules, marinades and instant soups often combine yeast extract, flavour enhancers, acids, tomato and spice. A plain or homemade version is easier to interpret.
Are preservatives hidden IC triggers?
IC-specific evidence for preservatives as a group is weak. Examples include potassium sorbate (E202), sodium benzoate (E211), sulphites (E220–E228), sodium nitrite (E250) and potassium nitrate (E252).
A reaction to a preserved food does not prove the preservative was responsible. Compare fresh meat with cured meat, plain yoghurt with a flavoured long-life product, or homemade sauce with a bottled sauce.
People with diagnosed sulphite sensitivity, asthma reactions or food allergy need medical advice. IC tracking is not a substitute for allergy assessment.
Products where hidden triggers often cluster
Flavoured water and electrolyte drinks: acids, vitamin C, sweeteners, carbonation and fruit concentrate.
Herbal tea blends: citrus peel, hibiscus, berries, liquorice, spices and flavourings.
Plant milks and protein drinks: cocoa, coffee, sweeteners, vitamin blends and acidity regulators.
Sauces, pesto and hummus: lemon, vinegar, tomato, chilli, mustard and preservatives.
Flavoured crisps and snacks: tomato powder, vinegar powder, chilli, MSG and yeast extract.
Gummies and dissolvable supplements: citric acid, ascorbic acid, sweeteners and flavourings.
Processed meats: smoke flavouring, spice, vinegar, nitrates, nitrites and flavour enhancers.
A five-minute label check
Does the product contain one of your already confirmed triggers?
Does it combine several common trigger categories?
Is acid added even though the food does not taste obviously sour?
Is it sugar-free because sweeteners were added?
Does the seasoning contain tomato, chilli, citrus or vinegar?
Is MSG or another flavour enhancer present?
Can you compare it with a plain or homemade version?
Do not reject a product just because its ingredients look scientific. Focus on repeatable reactions that match your own pattern.
How to test a packaged food
Choose a stable day and photograph the exact label. Begin with a small portion while keeping the rest of your food, supplements and activity familiar. Track pain, burning, pressure, urgency and frequency through the following morning.
Then compare a simpler version. If flavoured yoghurt appears to trigger symptoms, test plain yoghurt. If seasoned chicken causes symptoms, compare it with plain chicken. Repeat an unclear result before deciding that an ingredient is a trigger.

Practical swaps
flavoured yoghurt → plain yoghurt with tolerated fruit
bottled dressing → olive oil and herbs
tomato sauce → mild white sauce or vegetable purée
seasoning packet → individual herbs and salt
flavoured crisps → plain crisps or crackers
lemon hummus → customised hummus without citrus
marinated meat → plain freshly cooked meat
fizzy diet drink → still water or a tolerated drink
gummy supplement → an unflavoured formulation when appropriate

What not to do
Do not remove every E-number, eliminate whole food groups because of one packaged product, or change food, supplements and medication simultaneously. Do not treat a symptom diary as a food-allergy diagnosis or remain on a severely restricted diet without dietetic support.
Remember that constipation, hormones, pelvic-floor tension, stress, poor sleep, infection and portion size can also affect symptoms. If every food seems to trigger you, speak with a clinician and registered dietitian rather than continuing to reduce your diet.
Frequently asked questions
Is citric acid a common IC trigger?
Some people suspect it, and acidic foods and drinks are commonly reported triggers. IC-specific evidence does not prove that E330 triggers everyone. Test the exact product against a version without added acid.
Are citric acid and vitamin C the same?
No. Citric acid is E330. Ascorbic acid is vitamin C and is usually labelled E300. They are chemically different and may not affect you in the same way.
Are all E-numbers bad for IC?
No. An E-number identifies an authorised food additive. It does not mean the ingredient is toxic or an IC trigger.
